• FASA Member (Certified Surgical First Assistant) - $100
• Associate member $100.00 (practicing first assistants who are not certified)
• Affiliate member $100.00 (Physicians, Surgical Technologists, corporate and other supporters) – $100
• Retired /Disabled Member - $80.00
